Uninsured Forms & Services
Patients are encouraged to inform front desk of any forms for doctor to fill out while booking appointments.
Patients are required to complete patient section information. Forms may be completed at a later date.
Not all services are covered by OHIP. Occasionally a patient may require a service that is uninsured. Examples of uninsured services are sick notes, insurance forms, government forms, employment physicals, travel health, tray fees for procedures, etc. Please refer to the fee guide. All payments for uninsured services are to be completed at point of service.
